Impact of lipodystrophy on the experience of stigma and psychological well-being in people with HIV in Spain
Resumen
Background: To analyze the causal relationship between perception of lipodystrophy, perceived discrimination and internalized stigma, and psychological well-being in people with HIV in Spain. Methods: A cross-sectional, correlational study. People with HIV from all over Spain were administered a questionnaire to measure the following variables: the degree of perceived lipoatrophy and lipohypertrophy, perceived discrimination and self-exclusion behavior (UNAIDS stigma index, 2008), psychological well-being (GHQ-12), and subjective wellbeing (PANAS). These scales had a Likert-like response format. A total of 706 questionnaires valid for analysis were collected. To analyze the hypothesized causative model, structural equation modeling was performed (LISREL 8.7). Several absolute and relative indices are indicated to check fitness of the model: Chi-square test, GFI, AGFI, RMSEA. Results: Mean age of the sample was 43.1 years (SD=7.6). 70.8% were male, 28.2% female, and 1% transgender. 88% were taking antiretroviral therapy. Descriptive analysis showed a high degree of perception of lipoatrophy and lipohypertrophy in 35% and 25% respectively. With regard to psychological health, mean score was 16.71 (SD=7.6), suggesting a higher occurrence rate of mental illness as compared to a normal population (mean scores in the scale higher than 14 indicate disease). As to subjective well-being, negative affectivity predominated (M=2.48) over positive affectivity (M=2.23, 5-point Likert scale). Results of the hypothesized causative model showed lipodystrophy to be related to a greater perceived discrimination and self-exclusion, leading to an increased negative affectivity and a decreased psychological well-being ( 2(3) = 13.62, p < 01; CFI = 1; AGFI = .99; RMSEA = .07) Analysis by gender showed that women perceived more lipohypertrophy and lipoatrophy, suffered stigma to a greater extent, and had poorer psychological health as compared to men. Conclusion: The study draws significant conclusions regarding the negative impact of lipodystrophy on both experience of stigma and psychological health in people with HIV. (Resumen de los autores)
